New Account Credit Policy: Applying for credit requires a completed and approved new account application and may take up to 30 days to approve and extend credit. During this process all advertising will be pre-paid by check, cash or credit card. IF APPLICABLE, attach current 513 form.

The undersigned agree (jointly and/or individually) to guarantee payment of all charges which are due upon receipt. This includes a 1.5% finance charge on past due balances. In the event that the publishing company must engage the services of an attorney or collection agency to collect any sums owing pursuant to this agreement or to otherwise enforce or defend its right under this application, then the publishing company shall be entitled to recover against the advertiser or agency all costs incurred by it, including reasonable attorney fees.
